
A recent hearing screening or audiological evaluation is required prior to a speech-language evaluation. If your child has not had one, please obtain one before proceeding.

Your child's doctor may be able to conduct a hearing screening. If not, you may contact Fulton County Board of Health – Hearing Program Services: Diagnostic evaluations, Auditory Brainstem Response (ABR) testing, behavioral hearing testing, and Early Hearing Detection and Intervention (EHDI) services.

How to Get Started

OBTAIN A REFERRAL

Contact your child’s doctor for a signed referral for a speech therapy evaluation and treatment. The referral must include ICD-10 codes and must be signed.

SUBMIT ALL DEVELOPMENTAL/EDUCATIONAL
DOCUMENTS

Complete all intake paperwork and submit any relevant developmental or educational records. These may include:

Developmental evaluations
This information helps us prepare for your child’s evaluation and determine service needs.

INTAKE FORM
All documents may be uploaded and submitted through the registration form. Once everything is received, our office will contact you directly to schedule your evaluation. If no appointments are available, your child will be added to the waitlist.
